  • Account Transfer Form

    Transfer assets from an account at another firm into your Scottrade Account.

  • ACH Distribution Request

    This form should be used by the account owner(s) to request that a recurring distribution be sent to his/her checking/savings account using the ACH (Automated Clearing House) system.

  • Advisory Fee Invoice

    Please use to process your advisory management fees

    Once completed, mail or fax to your dedicated team

  • Alternate Payee/Address Check Request

    This form should be used to have checks issued to an alternate payee or address from a Scottrade account.

  • Authorization to Wire Brokerage Funds

    Wire funds out of a Scottrade account to a bank or other financial institution. A $25.00 fee applies to domestic wire transfers and a $40.00 fee applies to international wire transfers.

  • Authorized Agent Form

    This form should be used to add/remove individuals* (authorized traders, individuals with information access only and broker-dealer affiliates) to/from accessing your master account. This form should also be used if an authorized individual’s name has changed. Please contact your Relationship Team if the individual is a member/officer of the firm. Additional paperwork may be required.

  • Calculation for Required Minimum Distribution

    This form is used to calculate a Required Minimum Distribution (RMD) for tax purposes. A distribution request will need to be submitted in order to take the RMD.

  • Change of Address Form

    This form should be used to change the mailing and/or home (physical) address of one or more Scottrade accounts.

  • Charitable Gift LOA

    The Charitable Gift LOA authorizes the transfer of stock from your account to a charitable organization.

  • IRA Deposit Slip

    This form is only required for IRA deposits.

  • Letter of Authorization to Transfer a Contribution

    Contribute to an IRA or ESA account by making a transfer from another Scottrade account.

  • Letter of Authorization to Transfer Cash and/or Securities

    This form should be used to transfer cash and/or securities to another Scottrade account.

  • Letter of Authorization to Change Mutual Fund Dividend and/or Capital Gains Options

    Complete this form to change current dividend and/or capital gain options.

  • Margin Application

    Add margin trading to an existing account. Trading on margin is the practice of buying securities with money borrowed from the brokerage firm.

  • Mutual Fund Systematic Trade Authorization

    Establish periodic purchases or redemptions for mutual funds; a separate form is required for each purchase or redemption.

  • Mutual Fund Systematic Trade Cancellation

    Cancel systematic purchases or redemptions; a separate form is required for each purchase or redemption being cancelled.

  • Power of Attorney Affidavit

    Use this form to apply a Power of Attorney (POA) to an existing Scottrade account.

  • Signature Page

    For accounts opened online, a physical signature is required to perform certain account activities such as wire transfers, IRA distributions and account linking.

  • W-9 Form

    The W-9 form requests a taxpayer identification number and certification.
