Always use a username. When adding a friend on the 3DS, it gives you whatever name they have selected. I already have 2 Ryans with only 15 friends and no its not the same person.

When someone lets me know they have added me do I need to add them back or should I get some sort of notification to accept?

That is up to you.
If you do NOT add them back, neither one will appear on each other's friends list.
I don't think the 3DS notifies you when you've been added to someone else's friend list. It WILL notify you when both have entered each other's friend code and they will pop up on yours friends list with a Mii.
You can always delete friends, disable them from seeing when your online, and disable them from seeing what your playing.
I have online and games I'm playing enabled for friends to see.

^^ Thanks a lot. Nintendo seem to do these sort of things a little bit differently, is taking a little getting used to.
EDIT: Just wanted to add a general question that I didn't particularly want to make a new thread for. I don't really have an 'online' game for the 3DS yet and was wondering if Mario Kart 7 is a good pick, are there lots of people playing it?. I used to love Mario Kart on my N64.
